But I Hate Using a Ladder!

Roof Fall in Normal.

A roofer doing insulation removal on an old retail store fell 16 feet into an unguarded roof opening requiring hospitalization. OSHA requires roofing openings to be covered, marked, and be secured from movement. 

Logging Fatality

A worker in Clinton County was killed by falling a tree he was felling with a chainsaw. It was a rotted hollowed ash tree. The accident is under investigation. 

Aerial Lift Accident 

In Peoria, a worker was injured when moving his lift down an embankment. The lift slid, hit the bottom, and threw him out of the lift. OSHA requires all workers in aerial lifts to wear fall arrest.

Two workers were injured when the 15 foot deep trench collapse sending them to the hospital. 

Soil Classification

If Classifying soil, a manual and a visual test must be performed if classifying it as A or B. Previously disturbed soil is never Class A. Here is a review of the soil classifications for excavation operations. 

"Type A" means cohesive soils with an unconfined, compressive strength
of 1.5 ton per square foot (tsf) (144 kPa) or greater. Examples of
cohesive soils are: clay, silty clay, sandy clay, clay loam and, in some cases, silty clay loam and sandy clay loam. Cemented soils such as caliche and hardpan are also considered Type A. However, no soil is Type A if:
 (i) The soil is fissured; or
 (ii) The soil is subject to vibration from heavy traffic, pile driving, or similar effects; or
 (iii) The soil has been previously disturbed; or
 (iv) The soil is part of a sloped, layered system where the layers dip
into the excavation on a slope of four horizontal to one vertical (4H:1V)or greater; or
 (v) The material is subject to other factors that would require it to be classified as a less stable material.
 "Type B" means:
 (i) Cohesive soil with an unconfined compressive strength greater than
0.5 tsf (48 kPa) but less than 1.5 tsf (144 kPa); or
 (ii) Granular cohesionless soils including: angular gravel (similar to
crushed rock), silt, silt loam, sandy loam and, in some cases, silty clay loam and sandy clay loam.
 (iii) Previously disturbed soils except those which would otherwise be
classed as Type C soil.
 (iv) Soil that meets the unconfined compressive strength or cementation requirements for Type A, but is fissured or subject to vibration; or 
 (v) Dry rock that is not stable; or
 (vi) Material that is part of a sloped, layered system where the layers dip into the excavation on a slope less steep than four horizontal to one vertical (4H:1V), but only if the material would otherwise be classified as Type B.
 "Type C" means:
 (i) Cohesive soil with an unconfined compressive strength of 0.5 tsf (48 kPa) or less; or
 (ii) Granular soils including gravel, sand, and loamy sand; or
 (iii) Submerged soil or soil from which water is freely seeping; or
 (iv) Submerged rock that is not stable, or
 (v) Material in a sloped, layered system where the layers dip into the
excavation or a slope of four horizontal to one vertical (4H:1V) or
steeper.

In the past 12 months the Peoria OSHA office has conducted 74 roofing inspections in Central Illinois, averaging $1,625 in fines per inspection.  
In an effort to educate those in the roofing industry on the OSHA Standards, and prevent death or serious injury, the Peoria OSHA office would like to invite you to an informational session to review the OSHA standards that apply to the industry.  Your jobsites will not be targeted by OSHA for your attendance to this session; in fact the meeting will provide you with the information and materials that you need to be in compliance with many OSHA Standards, if followed.
The FREE informational meeting for Roofing Contractors in the Tazwell County Area will be held on Tuesday, December 20, 2005 from 4:30 – 5:30 PM, at Illinois Central College, East Peoria Campus, Agricultural & Industrial Technologies Building, Room 203.  Please park in parking Lot F.

Z Project Conference, May 2, 2006, Lifesaver Award, for a Lifesaving Hazard Control



In a continuing effort to share the hazard controls from the Z Project Conference, here is the information on the recipient of the Z Project Lifesaver Award, which recognizes hazard control(s) which could actually have saved a life and/or lives.  Marathon Petroleum Company LLC in Robinson, IL incorporated fall protection elements into the building of their Gasoline Desulfurization Unit (GDU), to ensure the safety from fall hazards of the workers and operators on and around the vessels.  The following photos show the old and new systems and incorporated the following:

Stairs were installed on towers in addition to fixed ladders.  Stairs are easier to carry tools or to wear self-contained breathing apparatus when climbing.  Fixed ladders are much more common in the refining industry for accessing towers.  Fixed ladders require the materials to be hoisted in often awkward positions.  Also, climbing fixed ladders with a self-contained breathing apparatus is cumbersome.

Permanent fall arrest equipment was installed instead of relying on portable fall protection equipment.  The permanent system reduces possible entanglement hazards while working/walking.

Catwalks were installed for easier access/egress to and from adjacent vessels.  These catwalks reduce personnel going up and down stairs or ladders, often carrying equipment. 

The GDU construction site was managed by Fluor (site contractor) with their “Managing to Zero” observation based safety program.  The job site achieved a performance level of zero OSHA recordables.  Fluor awarded the project construction site their prestigious “Tri Star Status” for the 537,000 hour construction performance in the GDU.  

There were also zero OSHA recordable injuries by Illinois Refining Division employees who worked 169,710 manhours during the construction of the GDU.

New Partnership – P.J. Hoerr, OSHA and Onsite Consultation

On July 7, 2006, OSHA and the Illinois Onsite Safety and Health Consultation Program entered into a Strategic Partnership with P. J. Hoerr, Inc. during the construction of the Washington Area Community Center in Washington, Illinois.  This project is expected to be completed in a year and a half.  Welcome to P. J. Hoerr, Inc. to our partnership family and raising the bar on safety and health performance at the worksite!!!  Thanks to Ken Koroll, Safety Specialist for his work in forming the partnership!


Underground Focus Live!  August 22nd to 24th, 2006

In conjunction with our Alliance with JULIE, OSHA began interacting with the Underground Focus Live last year.  The event provides insight to contractors performing trenching and excavation work with actual on-site demonstrations and related training.  Information about the event can be found at www.UndergroundFocusLive.com.  This year, OSHA will actually be performing some mock inspections at the open trenches on-site.  This is your opportunity view what we document and ask questions of the OSHA compliance officer.  You can find information on the session on the above website under “Demonstrations” or go to http://www.infrastructureresources.net/email_OSHA.html.  In addition, the Chicagoland Construction Safety Council will be having a hands-on Competent Person Training Course.  The information is at the Underground Focus Live website above under “Education” or you can use the direct link of http://www.infrastructuresresources.net/email_excavation.html.  OSHA and Illinois Onsite Safety and Health Consultation will also have booths at this event.  Underground Focus Live! will be at the Chicago Speedway in Joliet, Illinois.

Peoria Changes
Just to let you know, our Area Office has a new Acting Area Director, Frank J. Winingham, from our Madison, Wisconsin Area Office.  Frank will be with us for the month of August.  Frank’s area of expertise is General Industry.  In addition, he is a certified instructor on the topic of Workplace Violence.  He is on the National Specialized Response Team for Weapons of Mass Destruction.  Finally, he was part of the OSHA teams who responded to assist at the World Trade Center and to New Orleans/Katrina catastrophes.  Welcome Frank!!!  We are still waiting for a new Area Director.  We want to thank Bill Donovan, who returned to our Regional Office at the end of July, for his help in managing our office.




Boiler Explosion Accident

Decatur, IL – Two employees and a contract worker were severely burned with one fatality.   A boiler door exploded emitting several thousand gallons of hot liquid at a syrup manufacturing facility.  The case is currently under investigation.

CPR/AED Training for OSHA Staff

In conjunction with our Alliance with Richland Community College, Andy Perry provided the updated American Heart Association CPR/AED training to the Peoria Area Office OSHA staff.  Our AED has been strategically placed in our office in Peoria.  Here is Andy with our OSHA staff.

Emergency Heat Illness Regulation Made Permanent In California Has Relevance Anywhere It’s Hot

On April 20, 2006, California's occupational safety and health officials at Cal/OSHA made permanent the emergency heat-safety workplace standards.  These regulations, intended to protect workers from heat illness, were implemented as a temporary emergency measure in the late summer of 2005, after several on-the-job heat-related fatalities.  While these temporary regulations were in effect, a Cal/OSHA advisory committee worked on a permanent standard for the state, to provide ongoing protection to workers during peak temperature periods. These permanent standards were formally announced on April 20 by Cal/OSHA. The new permanent heat-safety regulation covers four critical areas to protect workers:
  • Access to adequate supplies of cool water
  • Shade for workers who are on the verge of becoming ill or already have started to feel the effects of heat illness
  • Training on critical elements of heat illness prevention
